Where can I purchase a detailed record of my work histroy?

I'm looking for a form online, but can't find anything. I'm trying to finish up an application but I can't recall all the jobs I've had, let alone the length in which I worked them. I'd rather not have to make stuff up.

Where have you been? Did you just one day decide to "check out" and finally returned? Anyway, you should remember where you worked. They can probably help you with the details.
