Who can I go to re: unfair/inappropriate office behaviour when my office manager does nothing?

Office manager plays favorites, does not enforce office policies, no repurcussions for not doing job/neglect of performance, office policies change on the whim of the office manager, unprofessional behaviour and language (foul) are allowed. This is a medical office setting - we are losing patients and the doctors have their heads in the sand and believe the lying office manager.
