Four and a half years have passed since High School graduation. Lucas is now coaching the Ravens with the assistance of Skills and is in a relationship with his book editor, Lindsey Strauss (guest star, Michaela Mc Manus), who urges him to write a second novel that will match the success of his first. Nathan and Haley are raising their son Jamie (Jackson Brundage) in Tree Hill, where an accident four months previous left Nathan near-paralyzed and his basketball dreams diminished. Haley is a teacher at Tree Hill High while Mouth is an unemployed sportscaster looking for work. Brooke is undoubtedly the most successful of the group, having turned Clothes Over Bros into a multi-million dollar company, which she is running from New York. Peyton is in Los Angeles working as "the assistant's assistant" at a record company. Feeling undervalued and disrespected, she quits and after talking on the phone, she and Brooke realize that they will never be as happy as they were in Tree Hill so they return home together.
In Tree Hill, Brooke opens a Clothes Over Bros store in Karen's Café and buys a new house for herself and Peyton. Her company CEO, later revealed to be Brooke's over controlling mother Victoria, (guest-star Daphne Zuniga) comes in to town and tries to get things back under her control, undermining Brooke and her friends. Brooke begins dating the new bartender, Owen (guest star Joe Magniello), and as a result of his constant mocking of her doing nothing, she takes him to New-York where they stumble upon Rachel who has OD'ed in an apartment Brooke just bought to expand. They revive her and take her to Tree Hill, to get her life back on track, but Victoria lashes at her and Rachel leaves for parts unknown with Brooke's money. Fed up, Brooke fires Victoria and begins to work on having her own family. Turned down for adoption, she is given temporary custody of Angie, a little girl who needs special surgery, leading Owen to break up with her, using his good friend Chase, Brooke's clean teen ex, as the messenger. Through her struggles with being a mother and taking care of the baby, she gets closer to Lucas and is heart-broken when she has to return her.
Peyton starts her own record label, funded by Brooke, and finds her first artist Mia (guest star Kate Voegele) in a group with a cocky leader (guest star Kevin Federline), whom she fires. With the help of Haley, who produces the record, and a ploy to get Peyton's ex, Lucas, to fund her studio space, Mia becomes a star and on tour eventually meets someone who claims to know Peyton's mother Ellie. Is it her father? Meanwhile she also struggles with her feeling towards Lucas, creating some animosity between her and Lindsey, who is also Lucas' fiancée, whom he began dating after having broken up with Peyton and missed a chance at reconciliation. Indeed, it is thanks to Peyton that Lucas, who is now the Ravens' coach with Skills, overcomes his writer's block. They eventually kiss but he proposes to Lindsey the same night. However after reading his new book about a boy who falls in love with a comet, which she takes as a metaphor for Peyton's Comet car, she dumps him at the altar, leaving a distraught Lucas. After some advice from Andy, who has gotten back together with Karen, Lucas tries to get Lindsey back but is turned down as she believes he still loves Peyton. Later on, out of frustration with learnig Lindsey is dating, Lucas blows up on a player of the rival team, and loses his job. He turns to alcohol but eventually comes back into the light with the help of Nathan and Haley. Peyton, in her own way, lets Lucas know that she will always love him, while Lindsey leaves a message saying that she still loves him and was just afraid of the immensity of his love.
Fed up with Nathan's complaining and busy with her producing, Haley hires a nanny (guest star Torrey DeVitto) who then tries to insert herself in their life, eventually kissing Nathan and kidnapping Jamie during Lucas' wedding, but is stopped and threatened by Dan, who has just been released from prison. With the help of the nanny, Nathan had sorted out his life and took the new Ravens' lead player, Quentin (guest star Robbie Jones) whom he likens to his former self, under his wings. When the nanny flirting was revealed, Nathan and Haley's marriage almost suffers its last blow but thanks to some counseling, they sort things out and get back together and decide to pursue their individual dreams, Haley making a new record with Peyton, and Nathan getting back to basketball. Dan then reveals he is dying from his HCM but all his family turns his back on him, especially Deb, who is now Jamie's nanny, and Nathan. However, he tries to see Jamie, who admires Dan for saving him. Haley finally relents and lets Jamie give him a thank you card, although she clearly states she didn't do it for Dan.
Mouth is unemployed but finally finds himself a job at the archiving department at a local TV station. He begins an unexpected affair with his boss, Alice (guest star Kelly Collins Lintz) which kick starts his career but he begins to feel bad about it and starts dating Brooke's assistant Milicent (guest star Lisa Goldstein), under Alice's jealous eye who fires him. However, he manages to keep his job when at the last minute another office affair involving Alice is exposed, as she was sleeping with many employees. He is given an on air position but loses it after refusing to air Lucas' blow-up. Milicent, who has recently moved in with him in the apartment he shares with Skills, Junk and Fergie, convinces him to accept an offer in Omaha after quitting his job in Tree Hill. Skills, who has broken up with Bevin, who is now married to Tim, with whom she has a child, begins dating a woman online, who is eventually revealed to be Deb and they start an affair.
As the season ends, Dan becomes number 1 on the donor's list after the death of the one before him after having contemplated killing him but deciding not to for Jamie. He is ironically run over just before his beeper goes off to tell him a heart is ready for him. Mouth quits his job to go to Omaha and Nathan has his first success in his comeback, scoring against Quentin, inspired by his son, who had overcome his own fear of swimming after a near-death experience. Lucas, confronted by Haley, calls someone from an airport, asking her if she wants to get married in Vegas that night. The viewers are left wondering who it is, as Peyton, Brooke, and Lindsey all get a phone call at the same time.
