MTF? help? I want to become a woman.?

I am embarassed to say this but.... I am a 16 year old male and I sometimes feel that i would be happier if I were born a girl. I will usually wear thongs/panties/bra's and try on my sisters clothes in a secretive manner. I enjoy wearing the clothes and I will occasionally sleep with them on while wearing sweats or a sweatshirt over them to conceal them from people looking at me. When I think of sexual desires, I would much rather prefer it from the female's point of view. I want to have a functioning v****a and be able to pass 100% completely as a woman. Are these my hormones acting crazy? or should I have really been born female? I am scared that if I become a woman people will know I used to be male.
