Who can extend job offer? Is it only done by the HR manager?
If applicant interviewed with dept manager and HR manager, can the dept manager extend job offer or does it have to be done by the HR manager? Also, if the HR manager is not available, can the dept manager extend offer from approval of HR? Or does it need to be done by the person next in line to the HR manager such as Operations? Cannot find information online.

Not just anyone can offer a job. It depends on the structure of the company and every company is different. It seems likely that if someone offered a job, they have the authority to do so. But you can always call and check with someone else at the company if you are skeptical.
