Should I include the address of where I worked in a Resume?

I have always just filled out applications for jobs. This time I was told to bring in my resume for the job interview tomorrow. I am making one using microsoft word. Do I put in the address, phone numbers, supervisors of past jobs?

When making a resume I was taught in college you do know disclose to much information. I was required to have the name of the business the address and what your duties were as well as the dates you worked. if they want anything else then you provide it. and do not include references if they ask for them then you... For a resume, the company name, city and state are sufficient. But bring the additional information with you. You may need it.
